• Duration: 13 weeks with potential extension up to 6 months
• Shifts: 5 days/week, 8-hour day shifts (8:00am – 4:00pm; may be adjusted)
• On-Call: 24/7 after-hours on-call coverage required
• Estimated Weekly Pay: $3,233 – $3,592
• Team Size: Oversight of 13 FTEs (case managers, resource managers)

Key Qualifications

• Bachelor's degree in Nursing required; Master’s degree in Nursing, Health Administration, or related field preferred
• Active RN license in good standing (Arizona RN license preferred or compact license eligibility)
• 3–5 years of acute care management experience, demonstrated success in utilization management and case management
• 2–5 years of leadership experience as a Director or Manager of Care Management, Case Management Director preferred
• Strong knowledge of regulatory requirements, compliance, and quality improvement in acute care settings
• Excellent communication, organizational, and team leadership skills; experience supervising case managers and resource managers
• Ability to provide two professional references

Core Responsibilities

• Oversee the implementation, development, and daily management of Care Management and Resource Management programs
• Lead discharge planning, utilization review, and care coordination to support efficient transitions of care and reduce readmissions
• Ensure compliance with regulatory requirements, payer policies, and hospital standards
• Establish and optimize workflows, policies, and metrics for patient-centered care coordination and population health initiatives
• Provide leadership, mentorship, and professional development for a team of case managers and resource managers (13 FTEs)
• Collaborate with physicians, nursing leadership, social work, care coordinators, and community partners to achieve optimal patient outcomes
• Monitor department performance, ROI, quality improvement projects, and utilization metrics; report outcomes to executive leadership
• Participate in after-hours on-call rotation as needed

• Experience with electronic health records (EHR) systems and case management documentation
• Strong analytical skills and experience using data to drive quality improvement and cost-efficiency
• Proven ability to lead cross-functional teams and implement change in a fast-paced acute care environment
