Position Overview

As a Home Health Physical Therapist, you'll provide individualized therapy services to patients in their home environment, helping them improve mobility, independence, safety, and overall quality of life.

You'll perform comprehensive evaluations, develop personalized treatment plans, implement evidence-based interventions, and collaborate with an interdisciplinary care team to achieve optimal patient outcomes while adhering to physician orders and regulatory standards.

Qualifications

• Master's or Doctorate degree in Physical Therapy from an accredited program
• Active Physical Therapist license or eligibility for licensure in Florida
• One to two years of home health experience preferred
• Strong clinical judgment, communication skills, and commitment to patient-centered care
