Residential Physical Therapist (PT) – Full-Time
Vero Beach, FL
Monday-Friday40 hrs/week | Salaried

Join an in-house rehabilitation team within a beautiful Continuing Care Retirement Community (CCRC), providing 1:1 physical therapy services to residents in their own spaces—all within a private campus setting. Position Overview
As a Residential Physical Therapist, you’ll provide skilled PT services to residents in their private homes—helping them maintain independence, improve mobility, and enhance overall quality of life. You’ll evaluate patients, develop individualized plans of care, deliver evidence-based interventions, and collaborate with the interdisciplinary care team to achieve optimal outcomes.

Primary Responsibilities
  • Evaluate residents and develop individualized PT treatment plans
  • Provide skilled interventions to improve strength, balance, mobility, gait, endurance, and functional independence
  • Reassess progress, modify plans as needed, and complete timely documentation and discharge planning
  • Collaborate with physicians, nursing, and the interdisciplinary rehab team to coordinate care
  • Educate residents/caregivers on HEPs, fall prevention, safe mobility techniques, and wellness strategies
 
Qualifications
Required
  • Master’s or Doctorate degree in Physical Therapy (accredited program)
  • Active Florida PT license (or eligibility)
  • Ability to meet Florida Care Provider Background Screening requirements
Preferred
  • 1–2 years of Home Health PT experience
  • Geriatrics experience
  • Strong clinical assessment and communication skills
