Home Health Registered Nurse (RN) – 13-Week Contract in Kissimmee, Florida
Join a dedicated home health care team as a Home Health Registered Nurse serving patients throughout Kissimmee and the Central Florida area. Provide skilled nursing care in patients’ homes, promote independence, and help each patient achieve meaningful health outcomes. - Start Date: October 19, 2026
- Contract Duration: 13 weeks, through January 17, 2027
- Schedule: Daytime shifts, approximately 8:00 a.m.–5:00 p.m.
- Shift Duration: 8-hour shifts
- Hours: Approximately 40 hours per week
- Weekend/On-Call Requirement: Weekend and on-call availability may be required
- Travel: Local travel between patient visits is required
Home Health Registered Nurse Requirements
- Active Registered Nurse (RN) license in the state of Florida
- Associate degree in Nursing or higher
- At least 1 year of professional nursing experience
- Previous home health nursing or visiting nurse experience required
- Experience with HCHB home health documentation and workflow strongly required
- Current professional nursing certification and credentials required for practice
- Strong patient assessment, organization, communication, and clinical documentation skills
- Ability to work independently in patients’ homes and manage a geographically distributed caseload
- Reliable transportation and the ability to travel throughout the assigned coverage area
Home Health RN Responsibilities
- Perform comprehensive skilled nursing assessments and develop individualized plans of care
- Complete start-of-care, resumption-of-care, recertification, and follow-up nursing visits as assigned
- Provide skilled nursing interventions, patient education, medication oversight, and disease-management support
- Monitor patient progress, identify changes in condition, and communicate concerns to the appropriate members of the care team
- Coordinate care with physicians, therapists, aides, families, and other interdisciplinary team members
- Document visits, assessments, interventions, and outcomes accurately in the HCHB system
- Support safe transitions of care and help patients and caregivers understand treatment plans
- Manage daily visits efficiently while maintaining high standards of patient-centered home health care
- Participate in required weekend and on-call coverage
