Job Requirements
- Active Registered Nurse (RN) license
- At least 1 year of nursing experience required
- Associate degree in Nursing required
- Previous home health, home care, or community-based nursing experience preferred
- Strong clinical judgment, time management, and communication skills
- Ability to work independently while maintaining a patient-centered approach
- Professional certification may be required based on assignment needs
Responsibilities
- Provide skilled nursing care in patients' homes in accordance with individualized care plans
- Perform assessments, monitor patient status, and report changes promptly
- Administer treatments and coordinate care to support optimal patient outcomes
- Educate patients and families on care plans, medications, and safety practices
- Document care accurately and maintain compliance with all clinical standards
- Collaborate with the broader care team to ensure continuity and quality of care
- Promote patient independence, comfort, and recovery in a home-based environment
