Home Health Registered Nurse Requirements
- Home health nursing experience
- Associate degree in nursing or equivalent nursing education
- Current BLS/CPR certification
- Homecare Homebase (HCHB) experience preferred
Home Health Nursing Responsibilities
- Conduct skilled nursing start-of-care visits in patients’ homes
- Complete OASIS assessments and develop individualized plans of care
- Assess patient conditions, identify changes in health status, and communicate clinical findings to the care team and providers
- Provide patient and caregiver education regarding medications, disease management, safety, and ongoing care needs
- Coordinate care with physicians, therapists, aides, and other members of the interdisciplinary team
- Maintain accurate, timely, and compliant clinical documentation
- Manage a field-based schedule across assigned service areas while meeting weekly productivity expectations of approximately 30 points
- Support safe transitions of care and promote positive patient outcomes in the home setting
