Job Requirements
- Active Registered Nurse (RN) license in hand
- Minimum of 1 year of adult Home Health experience or 1 year of Hospice experience, depending on specialty focus
- Current CPR/BLS certification
- Recent experience using HomeCare HomeBase within the last 6 months, or willingness to complete retraining if required
- OASIS proficiency required for RN candidates
- Reliable transportation, valid driver’s license, and auto insurance
- Comfortable traveling daily to patient homes and driving throughout a wide service area
- Strong organizational skills and ability to complete documentation within 24 hours
- Must be open to weekend rotation and on-call coverage as needed
- Ability to work independently while following agency policies and procedures
Responsibilities
- Provide skilled nursing care in patients’ homes based on physician orders and agency guidelines
- Assess, plan, implement, and evaluate individualized home health care using the nursing process
- Coordinate and direct care while communicating effectively with the clinical team
- Supervise and evaluate Home Health Aides and Licensed Practical/Vocational Nurses as needed
- Manage a varied caseload that may include Start of Care, wound care, and follow-up visits
- Travel between patient visits while maintaining safe, timely, and professional care delivery
- Complete all required charting and documentation accurately and within 24 hours
- Meet weekly productivity expectations and maintain a strong focus on quality outcomes
- Participate in periodic office meetings and coordinate care with interdisciplinary team members
- Remain flexible to support patient needs across the assigned territory
