- Schedule: Monday through Friday, 8:00 AM to 5:00 PM
- Shift Duration: 8-hour shifts
- Guaranteed Hours: 40 hours per week
- On Call: No
- Setting: Home health / hospice environment
- Travel Assignment: Yes
Job Requirements
- Active Physical Therapist license
- Oasis experience required
- HCHB experience required
- Home health experience strongly preferred
- Strong clinical assessment, documentation, and communication skills
- Ability to work independently while collaborating with an interdisciplinary care team
Responsibilities
- Perform comprehensive physical therapy evaluations and treatments based on physician orders and referrals
- Review patient history, diagnoses, precautions, and contraindications to guide safe and effective care
- Develop measurable goals and individualized treatment plans to improve patient outcomes
- Reassess progress regularly and adjust treatment plans as needed
- Communicate patient progress, needs, and discharge plans to the care team, physicians, and family members as appropriate
- Recommend adaptive equipment and incorporate its use to support patient independence
- Complete evaluation, treatment, and billing documentation accurately and on time
- Support and supervise PT assistants and aides in accordance with state practice standards
- Participate in staff development and continuing education to maintain strong clinical skills
