- Travel Radius: Up to 75 miles from the assigned care center

Job Requirements
- Minimum 1 year of recent adult home health experience required
- Current Physical Therapist licensure in hand
- Current CPR/BLS certification required
- Proficiency with HomeCare HomeBase EMR required
- Reliable transportation, valid driver’s license, and active auto insurance required
- Comfortable traveling between patient homes throughout the day, including longer drives between visits
- Ability to manage documentation within 24 hours of each visit
- Open to on-call rotation and flexible scheduling needs
- Must be comfortable working independently in a home-based care environment
Responsibilities
- Provide skilled physical therapy services in patients’ homes
- Complete evaluations, reassessments, and treatment visits based on the plan of care
- Develop and update short-term and long-term goals tailored to each patient
- Support patients recovering from illness, injury, or functional decline
- Document all visit activity accurately and promptly in the EMR
- Collaborate with the care team to help coordinate patient needs and outcomes
- Manage a daily schedule that may include 5-10 patients per day
- Travel between patient homes and attend periodic office meetings as needed
- Maintain professionalism, time management, and strong communication throughout the assignment
- Work toward weekly productivity expectations while delivering high-quality, patient-centered care
