Required Qualifications

• Active Physical Therapist (PT) license in the state of Missouri (Missouri PT license)
• Minimum 1 year of PT experience; new grads with Level II clinical experience considered
• Experience in Long-Term Care (LTC), Rehabilitation, or inpatient hospital settings (inpatient physical therapy, acute care PT)
• Strong knowledge of APTA guidelines, state practice acts, professional code of conduct, and ethics
• Proficient with standardized assessments, outcome measures, and electronic health record (EHR) documentation

Core Responsibilities

• Screen, evaluate, and develop individualized plans of care for patients with mobility, functional, and wound care needs
• Provide evidence-based interventions including manual therapy, therapeutic exercise, gait training, ambulation, and modalities
• Perform wound care interventions and collaborate with the interdisciplinary team on complex wound management
• Use standardized assessments and outcome measures to guide treatment and measure patient progress
• Document treatment plans, progress notes, and discharge summaries accurately in the EHR
• Adjust treatments based on patient response and clinical judgement, maintaining regulatory and clinical standards
• Provide patient, family, and caregiver education; consult and coordinate with nursing, physicians, and other rehab staff
• Participate in team meetings, trainings, and in-services to support quality improvement and clinical best practices
• Supervise and support PT assistants, rehabilitation aides, and students as needed
