Nurse Practitioner – Hybrid In-Home Chronic Care
Location: Story County, IA
Schedule: Full-Time | Monday–Friday | Hybrid (Field + Remote)

What Your Week Looks Like
Hybrid structure:

3–4 days conducting in-home visits
-
1–2 days remote for telehealth, documentation, and care coordination
Patient care model:

Approximately 4–5 patients per day

90-minute initial visits for comprehensive assessments

30–60 minute follow-up visits
Your day may include:

Conducting comprehensive in-home assessments

Managing chronic conditions and medications

Supporting hospital-to-home transitions

Educating patients and caregivers on disease management

Coordinating care with interdisciplinary teams
This model allows you to practice at the top of your license while building deeper patient relationships.
