Job Details

• Weekly Estimated Pay: $1,460 - $1,700 (competitive travel LPN pay)
• Location: Lexington, KY – Homecare / Home Health Nursing (serving 14 zip codes in Fayette County and 16 counties total)
• Hours per Week: 40 hours
• Shift: 8-hour days, Monday through Friday (in-home visits)
• On-Call: 1-2 days per week and 1 weekend every 4 weeks
• Contract Duration: 13 weeks (travel contract)
• Start Date: April 6, 2026
• Mileage Reimbursement: $0.43 per mile (travel LPN benefit)

Qualifications / Job Requirements

• Active LPN (Licensed Practical Nurse) license in Kentucky (LPN – KY)
• Minimum 1 year of LPN experience; homecare / home health nursing experience preferred
• Ability to meet productivity requirement of 32 points per week minimum
• Valid driver’s license, reliable transportation, and comfort traveling between patient homes
• Strong communication, organizational skills, and professional documentation practices

Responsibilities – Home Health Nursing

• Provide compassionate, patient-centered in-home care to patients across Lexington and surrounding Kentucky counties
• Administer medications, perform assessments, monitor patient health, and document care accurately in the chart/EHR
• Coordinate care with interdisciplinary home health teams, case managers, and primary providers to ensure optimal patient outcomes
• Educate patients and families on care plans, disease management, and safe home care practices
• Participate in on-call rotation and scheduled weekend coverage as required by the travel LPN contract
• Maintain timely travel between patient homes while ensuring thorough and compliant documentation
