Job Requirements
- Minimum two (2) years of bedside critical care clinical experience (critical care nurse / ICU experience preferred)
- Active Registered Nurse (RN) license
- BLS certification (required prior to first shift)
- ACLS certification (required prior to first shift)
- Experience in LTAC, Sub-Acute, Thoracic Surgery, Surgical Pathology, Pediatric PACU, or Pediatric OR preferred
Responsibilities
- Provide high-quality patient care following individualized nursing care plans in a Long-Term Acute Care setting
- Perform timely nursing assessments within 8 hours of admission and develop individualized care plans
- Document assessments, interventions, and patient responses accurately and in a timely manner
- Supervise Licensed Practical Nurses (LPNs), Certified Nursing Assistants (CNAs), and orientees; provide direction and clinical support
- Participate in patient rehabilitation programs, discharge planning, and transitions of care to ensure continuity
- Identify patient and family teaching needs and establish education goals
- Communicate changes in patient status to the interdisciplinary care team and escalate concerns as needed
- Promote patient and staff safety by adhering to hospital policies and evidence-based protocols
- Contribute to a positive unit culture, efficient workflow, and team building
- Assume charge nurse responsibilities as requested following orientation
- Pursue professional growth through continuing education and self-directed learning
