Job Requirements

  • Minimum two (2) years of bedside critical care clinical experience (critical care nurse / ICU experience preferred)
  • Active Registered Nurse (RN) license
  • BLS certification (required prior to first shift)
  • ACLS certification (required prior to first shift)
  • Experience in LTAC, Sub-Acute, Thoracic Surgery, Surgical Pathology, Pediatric PACU, or Pediatric OR preferred

Responsibilities

  • Provide high-quality patient care following individualized nursing care plans in a Long-Term Acute Care setting
  • Perform timely nursing assessments within 8 hours of admission and develop individualized care plans
  • Document assessments, interventions, and patient responses accurately and in a timely manner
  • Supervise Licensed Practical Nurses (LPNs), Certified Nursing Assistants (CNAs), and orientees; provide direction and clinical support
  • Participate in patient rehabilitation programs, discharge planning, and transitions of care to ensure continuity
  • Identify patient and family teaching needs and establish education goals
  • Communicate changes in patient status to the interdisciplinary care team and escalate concerns as needed
  • Promote patient and staff safety by adhering to hospital policies and evidence-based protocols
  • Contribute to a positive unit culture, efficient workflow, and team building
  • Assume charge nurse responsibilities as requested following orientation
  • Pursue professional growth through continuing education and self-directed learning
