Job Requirements / Qualifications

  • Active Registered Nurse (RN) license (state-appropriate RN licensure required)
  • BLS Certification (required prior to first shift)
  • ACLS Certification (required prior to first shift)
  • Epic EMR experience (experience with electronic medical records preferred)
  • Minimum 1 year of bedside clinical experience (2 years highly desired) — LTAC or acute care experience preferred
  • Experience with ventilator care, tracheostomy care, wound care, IV/central line management is a plus

Responsibilities

  • Perform comprehensive nursing assessments and develop individualized care plans for LTAC patients, including ventilator-dependent and complex care patients
  • Provide safe, trauma-informed, and evidence-based care consistent with LTAC best practices
  • Supervise and support Licensed Practical Nurses (LPNs), Certified Nursing Assistants (CNAs), and orientees
  • Document assessments, interventions, and patient responses accurately and promptly in Epic EMR
  • Participate in patient rehabilitation programs and interdisciplinary team conferences to coordinate long-term care
  • Initiate discharge planning and provide patient/family education from admission through transition of care
  • Promote and maintain patient and employee safety, adhering to hospital policies, infection control, and compliance standards
  • Communicate changes in patient status to the care team to ensure continuity and patient safety
  • Provide direction and training to ancillary staff, delegating appropriately and fostering a collaborative team environment
  • Contribute positively to team building and support a culture of continuous improvement
  • Engage in ongoing professional development and continuing education to maintain clinical competencies
