Job Requirements

• Active Registered Nurse (RN) license (state-appropriate)
• BLS Certification (required prior to first shift)
• ACLS Certification (required prior to first shift)
• Minimum two (2) years of bedside clinical experience
• ICU experience preferred — critical care, step-down, or prior LTAC experience helpful
• Ability to supervise Licensed Practical Nurses (LPN), Certified Nursing Assistants (CNA), and orientees

Responsibilities

• Perform comprehensive nursing assessments on all patients within 8 hours of admission and develop individualized LTAC care plans
• Identify patient and family teaching needs and goals; collaborate with interdisciplinary team to support rehabilitation and quality outcomes
• Document assessments, interventions, and patient responses accurately and in a timely manner
• Participate in patient rehabilitation programs, providing education and skilled nursing interventions to support recovery
• Initiate discharge planning and patient teaching beginning at admission to ensure safe transitions of care
• Communicate clinical changes in patient status to care providers to ensure continuity, safety, and effective care coordination
• Provide direction, training, and supervision to ancillary nursing personnel and support staff
• Maintain personal and environmental safety in accordance with hospital policies and LTAC standards
• Contribute to a positive team environment and participate in unit team-building and quality improvement activities
• Assume charge responsibility as requested after orientation (charge RN duties)
• Prioritize clinical activities for effective use of time and staff to meet patient needs
• Engage in ongoing professional development and continuing education relevant to LTAC nursing and travel RN roles
