Job Requirements
- Active Registered Nurse (RN) license (state-appropriate)
- BLS Certification (required prior to first shift)
- ACLS Certification (required prior to first shift)
- Minimum two (2) years of bedside clinical experience
- ICU experience preferred — critical care, step-down, or prior LTAC experience helpful
- Ability to supervise Licensed Practical Nurses (LPN), Certified Nursing Assistants (CNA), and orientees
Responsibilities
- Perform comprehensive nursing assessments on all patients within 8 hours of admission and develop individualized LTAC care plans
- Identify patient and family teaching needs and goals; collaborate with interdisciplinary team to support rehabilitation and quality outcomes
- Document assessments, interventions, and patient responses accurately and in a timely manner
- Participate in patient rehabilitation programs, providing education and skilled nursing interventions to support recovery
- Initiate discharge planning and patient teaching beginning at admission to ensure safe transitions of care
- Communicate clinical changes in patient status to care providers to ensure continuity, safety, and effective care coordination
- Provide direction, training, and supervision to ancillary nursing personnel and support staff
- Maintain personal and environmental safety in accordance with hospital policies and LTAC standards
- Contribute to a positive team environment and participate in unit team-building and quality improvement activities
- Assume charge responsibility as requested after orientation (charge RN duties)
- Prioritize clinical activities for effective use of time and staff to meet patient needs
- Engage in ongoing professional development and continuing education relevant to LTAC nursing and travel RN roles
