Qualifications & Job Requirements
- Active Registered Nurse (RN) license in the U.S. (state-specific license required prior to start)
- BLS Certification (required prior to first shift)
- ACLS Certification (required prior to first shift)
- Minimum 1 year of bedside clinical experience (2 years highly desired)
- Experience in LTAC, sub-acute, or similar post-acute/rehabilitation settings preferred
- Strong clinical assessment, documentation, and interdisciplinary communication skills
Key Responsibilities
- Perform comprehensive nursing assessments within 8 hours of patient admission
- Develop and implement individualized care plans in collaboration with patients, families, and interdisciplinary teams
- Provide direct patient care using the nursing process: assessment, planning, implementation, and evaluation
- Document assessments, interventions, and patient responses accurately and in a timely manner
- Participate in patient rehabilitation programs and initiate discharge planning from admission
- Communicate patient status changes to providers and ensure continuity of care during handoffs
- Supervise and provide direction to ancillary nursing personnel and support staff
- Promote patient and staff safety by adhering to facility policies, infection control, and best practices
- Contribute positively to unit teamwork, efficiency, and a patient-centered care environment
- Pursue professional growth opportunities and maintain required certifications
