Job Requirements

• Active Registered Nurse (RN) license (required for travel nurse assignments)
• BLS Certification (required prior to first shift)
• ACLS Certification (required prior to first shift)
• Minimum of 2 years of recent bedside clinical experience
• Strong assessment, documentation, and communication skills — proficiency in nursing assessments and clinical documentation

Responsibilities

• Perform comprehensive nursing assessments on all patients within 8 hours of admission and develop individualized LTAC care plans
• Collaborate with patients, families, and interdisciplinary teams (physicians, therapists, case managers) to identify teaching needs and measurable care goals
• Accurately document all assessments, interventions, and patient responses to support continuity of care and regulatory compliance
• Participate in patient rehabilitation programs and provide direct patient care using evidence-based nursing processes
• Initiate discharge planning and patient teaching from the day of admission to support safe transitions of care
• Communicate changes in patient status to the care team promptly to ensure patient safety and quality outcomes
• Supervise and provide direction to Licensed Practical Nurses (LPNs), Certified Nursing Assistants (CNAs), and orientees
• Promote patient and employee safety by adhering to hospital policies, infection control standards, and safety protocols
• Contribute to a positive team environment and participate in unit team-building and quality improvement activities
• Assume charge nurse responsibilities as needed after orientation
• Engage in ongoing professional development and continuing education to maintain clinical competencies
