Job Requirements
- Active Registered Nurse (RN) license (required for travel nurse assignments)
- BLS Certification (required prior to first shift)
- ACLS Certification (required prior to first shift)
- Minimum of 2 years of recent bedside clinical experience
- Strong assessment, documentation, and communication skills — proficiency in nursing assessments and clinical documentation
Responsibilities
- Perform comprehensive nursing assessments on all patients within 8 hours of admission and develop individualized LTAC care plans
- Collaborate with patients, families, and interdisciplinary teams (physicians, therapists, case managers) to identify teaching needs and measurable care goals
- Accurately document all assessments, interventions, and patient responses to support continuity of care and regulatory compliance
- Participate in patient rehabilitation programs and provide direct patient care using evidence-based nursing processes
- Initiate discharge planning and patient teaching from the day of admission to support safe transitions of care
- Communicate changes in patient status to the care team promptly to ensure patient safety and quality outcomes
- Supervise and provide direction to Licensed Practical Nurses (LPNs), Certified Nursing Assistants (CNAs), and orientees
- Promote patient and employee safety by adhering to hospital policies, infection control standards, and safety protocols
- Contribute to a positive team environment and participate in unit team-building and quality improvement activities
- Assume charge nurse responsibilities as needed after orientation
- Engage in ongoing professional development and continuing education to maintain clinical competencies
