Job Requirements & Qualifications

• Active LPN license in Pennsylvania (Licensed Practical Nurse)
• Minimum 1 year of experience in long-term care, skilled nursing, or similar LTC setting
• Proven ability to work independently and as part of a multidisciplinary healthcare team
• Strong communication, clinical documentation, and organizational skills
• Current CPR certification required

Key Responsibilities – Long-Term Care Nurse

• Provide high-quality nursing care to residents in a long-term care (LTC) facility
• Administer medications and treatments as prescribed; ensure safe medication administration
• Monitor, assess, and document resident health status and progress in electronic or paper charts
• Collaborate with RN staff, CNAs, therapists, and providers to support resident care plans
