Job Requirements

  • Active Vermont LPN license, or eligibility to obtain prior to start (Vermont LPN)
  • Long-term care / SNF experience preferred
  • Reliable availability for evening shifts and every-weekend schedule
  • Own transportation required for on-site attendance in Rutland, VT
  • Time-off requests (RTO) must be submitted in advance and approved

Responsibilities

  • Provide direct resident care in accordance with individualized care plans and facility protocols
  • Administer medications and treatments safely; monitor residents for effectiveness and adverse reactions
  • Perform focused resident assessments and promptly report changes in condition to RNs and providers
  • Accurate documentation of care, medication administration, and resident status (electronic and/or paper charting)
  • Collaborate with interdisciplinary team members—RNs, physicians, therapists, and support staff—to support resident well-being
  • Promote resident comfort, dignity, safety, and quality of life during each evening shift
  • Communicate professionally with residents, families, and clinical team members
