Required Qualifications
- Active, unrestricted LPN license in Pennsylvania (required)
- Minimum 1 year clinical experience as an LPN; long-term care, skilled nursing, nursing home, or geriatric nursing experience preferred
- Current BLS certification (American Heart Association preferred)
- Medication administration and documentation experience (med pass)
- Strong communication, teamwork, and resident-centered care skills
- Available to work scheduled evening shifts
Key Responsibilities
- Deliver high-quality nursing care to residents in a long term care / skilled nursing environment
- Administer medications and treatments safely and document according to facility policy
- Monitor residents’ health status, report changes to the interdisciplinary healthcare team, and assist with care planning
- Assist residents with activities of daily living, mobility, and comfort measures
- Maintain accurate, timely clinical documentation and electronic health records
- Support infection control, safety protocols, and resident rights
- Collaborate with RNs, CNAs, therapists, and other clinical staff to deliver coordinated care
