Job Requirements

• Active LPN license in Pennsylvania (or eligibility for compact licensure)
• Minimum 1 year clinical experience in long-term care, skilled nursing, or similar setting (LTC nurse experience preferred)
• Current BLS (Basic Life Support) certification
• Strong communication, documentation, and teamwork skills; experience with EMR charting a plus
• Compassionate, resident-centered approach with ability to work in a fast-paced LTC environment

Responsibilities

• Provide high-quality nursing care to residents in a long-term care/skilled nursing facility
• Administer medications and treatments as prescribed; follow medication administration protocols
• Monitor, assess, and document residents’ health status and promptly report changes to supervising RN/physician
• Collaborate with RNs, physicians, therapists, CNAs, and interdisciplinary team members to support care plans and resident outcomes
• Assist residents with activities of daily living (ADLs), mobility, and personal care while promoting dignity and independence
• Support admissions, discharges, care planning, and routine clinical tasks to maintain continuity of care
