Job Details
- Weekly Estimated Pay: $1,350 – $1,420
- Contract Duration: 13 weeks (March 16, 2026 – June 14, 2026)
- Setting: Home health & hospice facility — home health LPN & hospice LPN travel job
- Shift: 8-hour day shifts, Monday–Friday with occasional weekend rotation
- Guaranteed Hours: 40 hours per week
- Travel: Mileage reimbursement at $0.54 per mile

Job Requirements
- Active LPN license in West Virginia or valid compact state license
- Minimum 1 year of LPN experience; home health or hospice experience preferred
- Proficiency with OASIS and HCHB documentation systems
- Skilled in wound care, basic laboratory sticks, medication administration, and patient assessments
- Ability to work independently in community settings and manage multiple clients safely
- Strong documentation, communication, and time-management skills
- BLS from AHA or equivalent (if required by facility)
Responsibilities
- Provide skilled nursing care per each client’s plan of care, promoting dignity and independence
- Perform comprehensive assessments, monitor vitals, and report abnormal findings to the interdisciplinary team
- Administer medications, enteral feedings, oxygen, ostomy care, and other nursing treatments as directed
- Deliver wound care and other skilled nursing interventions following agency and clinical protocols
- Collaborate with PT, OT, and Speech Therapists to implement and document established routines and goals
- Maintain infection control practices and report communicable diseases per policy
- Accurately document nursing actions, progress notes, OASIS assessments, and outcomes in HCHB
- Order and track necessary supplies, medications, and feedings within scope of practice
- Respond to incidents, complete incident reports, and perform post-incident assessments
- Maintain and report medical equipment issues to ensure patient safety
