- Guaranteed Hours: 36 hours per week
- Shift: 12 Hour Days (6:00 AM – 6:30 PM)
Qualifications & Requirements
- Active RN license (required)
- Minimum 1 year of nursing experience (LTC / SNF experience preferred)
- Ability to work 12-hour day shifts
- Current BLS certification from AHA
- Strong communication, critical thinking, and teamwork skills
- EHR/EMR documentation experience preferred
Responsibilities
- Provide compassionate, high-quality resident-centered nursing care in a long-term care/skilled nursing facility setting
- Assess, plan, implement, and evaluate individualized patient care and care plans
- Administer medications and treatments safely and accurately per provider orders
- Collaborate with interdisciplinary team members (physicians, therapists, social services) to optimize resident outcomes
- Document patient information and care activities accurately and timely in the EHR
- Support residents and families with education, emotional care, and discharge planning as needed
