- Position: Home Health Physical Therapist (PT) – Travel / Contract
- Location: On-site in Lancaster, OH (43130) — Lancaster, OH home health job, Fairfield County, Ohio
- Contract Length: 13 weeks
- Start Date: April 6, 2026
- Shift: Days (5 shifts per week, 8 hours per shift)
- Hours per Week: 40 (32 guaranteed)
- Estimated Weekly Pay: $2,569 - $2,854
PT Job Requirements
- Minimum 1 year of experience as a licensed Physical Therapist (PT)
- At least 6 months of Home Health experience as a Physical Therapist
- Experience with OASIS and HCHB required for clinical documentation
- Current, active Physical Therapist license in the state of Ohio
- Current CPR/BLS certification
- Valid driver’s license, vehicle insurance, and reliable vehicle or access to public transportation for in-home visits
Home Health PT Responsibilities
- Provide high-quality in-home physical therapy services focused on mobility, independence, fall prevention, and functional goals
- Perform comprehensive patient assessments and develop individualized treatment and discharge plans
- Document assessments, treatment plans, progress notes, and OASIS data using HCHB to ensure accurate clinical records
- Coordinate care with interdisciplinary teams, physicians, nurses, patients, families, and caregivers
- Educate patients and caregivers on therapeutic exercises, use of adaptive equipment, safety strategies, and home modifications
- Maintain professional standards, comply with regulatory requirements, and ensure timely, accurate documentation for home health compliance
