Job Requirements
- Active North Carolina Physical Therapist license
- Doctoral degree in Physical Therapy or equivalent required
- Minimum of 1 year of Physical Therapy experience required
- Home health experience strongly preferred
- HCHB experience is a plus
- Strong clinical judgment, communication skills, and time management
- Ability to work independently while delivering patient-centered care
Responsibilities
- Provide home health physical therapy services to patients in the community
- Perform evaluations, develop treatment plans, and adjust care as needed
- Support patients with mobility, strength, balance, and functional recovery goals
- Document patient progress accurately and in a timely manner
- Coordinate with interdisciplinary team members to promote quality outcomes
- Travel within assigned coverage areas to deliver visits efficiently
- Maintain a professional, patient-focused approach in all home visits
