Job Description
Primary Care Nurse Practitioner – Hybrid Remote
Looking for an NP opportunity that offers flexibility, innovation, and the chance to make a real impact in chronic disease management? We have a few full time, permanent openings in the Salt Lake City area.
The Role This is a primarily telehealth-based position; most of your work will be done virtually, with the occasional option to see patients in person if they're local to you.
You'll be part of building something new: an innovative virtual and in-person care model focused on providing primary care for patients with (CKD) and related chronic conditions.
Primary Conditions you'll manage:
- Chronic Kidney Disease (CKD)
- Hypertension
- Diabetes
- Heart Failure
Schedule & Lifestyle
- Monday–Friday only
- Self-managed schedule
- No nights, no weekends, no on-call
Requirements:
- Minimum 5 years of NP experience (primary care, internal medicine, cardiology, nephrology, or case management)
- Experience managing chronic diseases (CKD, CHF, diabetes, hypertension)
- Active, unrestricted NP license in Washington State
- DEA license (or ability to obtain within 90 days)
- Current CPR/BLS certification
- Comfortable working across multiple EMR systems
- Strong computer skills — MS Word, Excel, Outlook
Preferred Qualifications
- Telehealth experience (strong plus)
- Value-Based Care or Population Health background
- Medicare Advantage risk documentation/coding experience
- Experience reducing avoidable hospitalizations through proactive, coordinated care
- Familiarity with palliative care discussions
