• Patient Ratio: 1:20
• Department Attire: Black scrubs or business casual
• EMR: Cerner (Cerner EMR experience preferred)
• Holiday Coverage: Required for holidays falling during contract

Job Requirements

• Minimum 4 years of RN experience in hospital care management, including discharge planning and care coordination
• Active Registered Nurse (RN) license for Indiana (attach required license and certifications with application)
• Travelers must reside more than 50 miles from facility (local candidates not accepted)
• Driver's license and permanent home address required for submission (address must match ID)
• Experience with Cerner EMR preferred

Primary Responsibilities

• Coordinate and manage patient care transitions, including timely and safe discharge planning
• Serve as an RN Case Manager collaborating with interdisciplinary teams (physicians, social work, therapy, nursing) to ensure optimal patient outcomes
• Maintain accurate, timely documentation in Cerner EMR and ensure compliance with hospital policies
• Manage a caseload with patient ratio of 1:20, providing individualized care management and follow-up planning
• Participate in department meetings and contribute to quality improvement and utilization management initiatives
• Provide coverage during holidays as required by contract
