Job Details

• Weekly Estimated Pay: $2,490 - $2,950
• Location: On-site, Williamsburg, VA (23185) and surrounding counties — Williamsburg, Yorktown, Gloucester, Hampton Roads, Poquoson, Charles City
• Healthcare Setting: Home Health & Hospice / Homecare nurse role
• Schedule: Monday – Friday, 8:00 AM – 4:30 PM
• Shifts per Week: 5 (8-hour shifts)
• Hours per Week: 40
• Contract Length: 13 weeks
• On-Call: Some on-call required as needed
• Mileage Reimbursement: $0.43 per mile
• Start Date: February 23, 2026

Job Requirements

• Active Registered Nurse (RN) license in Virginia or a Compact State
• Minimum 1 year of nursing experience required; home health or hospice nursing experience preferred
• Hospice experience is a must — previous Hospice RN or palliative care experience strongly preferred
• HCHB (Homecare Homebase) experience is a plus
• Excellent communication, documentation, and organizational skills (EHR/charting proficiency)
• Valid driver’s license and reliable transportation for travel to patient homes within assigned counties

Responsibilities

• Provide skilled nursing care and case management for homecare and hospice patients as a Home Health RN / Case Management Nurse
• Assess patients in the home setting and develop, coordinate, and implement individualized care plans
• Collaborate with interdisciplinary teams (physicians, social workers, therapists) to ensure optimal patient outcomes
• Educate patients and families on care plans, medications, symptom management, and end-of-life care when appropriate
• Accurately document patient care and progress using electronic health records (EHR/HCHB preferred)
• Travel to patient homes across Williamsburg and surrounding Virginia counties; participate in required on-call rotation
