- Job Title: Hospice RN Case Manager (Travel Nursing)
- Location: Colonial Heights, VA 23834 – serving Colonial Heights, Chesterfield, Tri-Cities, Hopewell, Petersburg
- Assignment Type: Travel, On-Site
- Start Date: December 1, 2025
- Assignment Length: 8 weeks
- Shifts per Week: 5
- Shift Duration: 8 Hour Days (8:00 AM – 4:30 PM, Monday–Friday)
- Hours per Week: 40
- On-Call: Some on-call required
- Estimated Weekly Pay: $2,750 – $3,200
RN Qualifications
- Active, unrestricted Virginia RN license or Compact State Nursing License
- Associate’s or Bachelor’s Degree in Nursing (BSN preferred)
- Minimum 1 year of hospice nursing experience
- Experience with HomeCare HomeBase (HCHB) electronic charting preferred
- Case Management certification a plus
- Strong clinical assessment, communication, and organizational skills
- Ability to analyze complex medical information and make sound decisions
Hospice Nurse Responsibilities
- Conduct comprehensive assessments of high-risk hospice patients in Colonial Heights and nearby communities
- Collaborate with healthcare providers to develop and implement individualized care plans
- Monitor and ensure cost-effective, appropriate levels of hospice care
- Coordinate non-medical support services, including housing and transportation
- Manage care transitions and work with physicians, social workers, and discharge planners
- Engage specialty healthcare resources for optimal patient outcomes
- Maintain accurate records of clinical, functional, and fiscal outcomes
- Promote health and illness prevention initiatives
- Advocate for patients, ensuring privacy and confidentiality
- Participate in on-call rotation as needed
