  • Hours per Week: 40
  • Shift: 5x8 hour days (Mon–Fri, 07:30–16:00); weekends off
  • Setting: Acute Hospital (Long Term Acute Care / Rehab / Skilled Nursing experience valued)
  • Average Daily Census: 30
  • Documentation System: EPIC (experience with EPIC preferred)
  • Float Requirement: Yes, must be willing to float to different units

RN Case Manager Job Requirements

  • Minimum 2 years of direct patient care experience in an acute care setting — inpatient RN experience required
  • Associate Degree in Nursing (ADN) required; Bachelor of Science in Nursing (BSN) preferred
  • Current California RN license in good standing
  • BLS certification (AHA) required
  • Case Management certification preferred (CCM or similar)
  • Hands-on experience in inpatient care coordination, discharge planning, and post-acute clinical payer authorization
  • Proficiency with InterQual Criteria and Milliman Guidelines for utilization review
  • Strong organizational, analytical, and written/verbal communication skills
  • Ability and willingness to float to different units as needed
  • Familiarity with regulatory and payer requirements: CMS, CPT coding, DRG, HEDIS, HIPAA, ICD-10, NCQA, OSHA, The Joint Commission, Worker’s Compensation
  • Prior travel nurse or case management experience strongly preferred; no first-time travelers

RN Case Manager Responsibilities

  • Coordinate inpatient care and lead discharge planning for assigned patients to ensure safe transitions to home or post-acute care
  • Perform utilization management, concurrent review, and quality assurance using InterQual and Milliman Guidelines
  • Manage prior authorizations, appeals, denials, and medical necessity reviews with payers and case managers
  • Ensure optimal clinical outcomes while managing cost-effective care and appropriate resource utilization
  • Maintain accurate, timely documentation in EPIC and communicate care plans to interdisciplinary teams
  • Support regulatory compliance and reporting requirements related to CMS, The Joint Commission, and other accrediting bodies
  • Foster positive relationships with physicians, nursing staff, social work, and external stakeholders to improve patient flow and satisfaction
