Job Requirements
- Minimum 2 years of experience as a Home Health RN or equivalent homecare nursing experience
- Experience with OASIS assessments and HCHB electronic documentation required
- Current Registered Nurse (RN) license in the state of Ohio (Ohio RN licensure)
- Current CPR certification (BLS preferred)
- Valid driver’s license, vehicle insurance, and reliable transportation or access to public transit for travel within Uniontown
Responsibilities
- Provide high-quality, patient-centered nursing care in patients’ homes, focusing on post-acute and chronic care management
- Perform comprehensive in-home assessments and develop individualized care plans using OASIS documentation
- Coordinate care and communicate with interdisciplinary teams, physicians, and family caregivers to achieve optimal patient outcomes
- Educate patients and families on disease management, medication administration, wound care, and self-care techniques
- Maintain accurate, timely electronic documentation in HCHB and ensure compliance with home health regulations
- Travel locally within the Uniontown area to see multiple patients per day as a Homecare RN / Travel Nurse
