- Weekly Estimated Pay: $2,190 – $2,540
- Location: Houston, TX 77058 (Clear Lake area)
- Setting: Hospice (On-Site, Community-Based)
- Start Date: June 1, 2026
- Contract Duration: 13 weeks
- Hours per Week: 40 (Travel Nurse schedule)
- Shifts per Week: 5
- Shift Duration: 8-hour days (Monday–Friday, 6:00 AM – 5:00 PM; 8 hours within this window)
- On-Call/Weekend Requirement: Yes, on-call required (hospice on-call rotation)
- Travel: Mileage reimbursed between patient visits (from first to last patient)
- Dress Code: Scrubs (any color)
Qualifications / Job Requirements
- Active Registered Nurse (RN) license in the state of Texas
- Associate Degree in Nursing (ADN) or higher; BSN preferred
- Minimum 1 year of nursing experience required; hospice nursing experience highly preferred
- Experience as a hospice RN case manager or in home health/end-of-life care is a plus
- Strong communication, documentation, and organizational skills
- Valid driver’s license and reliable transportation for community-based patient visits
Responsibilities – Hospice RN Case Manager
- Serve as the primary RN case manager for hospice patients, delivering compassionate end-of-life care and advocacy
- Perform comprehensive assessments and develop individualized hospice care plans
- Coordinate and lead interdisciplinary group (IDG) meetings and collaborate with physicians, social workers, and chaplains
- Provide symptom management, medication education, and caregiver support for patients and families
- Accurately document assessments, interventions, and communication in accordance with regulatory standards and hospice clinical policies
- Participate in on-call rotation and provide support during weekends as needed
