Job Requirements
- Active Licensed Clinical Social Worker (LCSW) license

Responsibilities
- Provide psychosocial assessments and behavioral health support for patients
- Develop and implement individualized care plans and discharge plans
- Coordinate services with clinical staff, patients, families, and community resources
- Offer crisis intervention and emotional support as needed
- Advocate for patient needs and assist with safe transitions of care
- Maintain timely, accurate, and compliant documentation
