We are seeking a compassionate and motivated Social Worker - LMSW for a 13-week on-site travel assignment in Albany, Oregon. In this hospice-focused role, you will support patients and families through important stages of care across home and community-based settings. Job Details
- Weekly Estimated Pay: $1670-$1800
- Employment Type: Travel Social Work Job

Job Requirements
- Oregon LMSW (Masters-level Social Worker) or CSWA licensure required
- Healthcare Provider Level BLS required
- Valid driver's license required
- Reliable personal automobile required for travel between patient care settings
- Auto liability and property damage insurance as required by law
- Vehicle must be maintained in good working order
- Previous hospice experience preferred
- Oregon LCSW preferred
- Experience as a social worker in a hospital or medical setting preferred
Responsibilities
- Provide medical social services to hospice patients and their families
- Support patients and loved ones through the emotional, social, and practical aspects of end-of-life care
- Collaborate with the interdisciplinary hospice care team to help develop and support individualized care plans
- Assist with care coordination across home settings, skilled nursing facilities, assisted living facilities, and adult foster homes
- Help connect patients and families with appropriate community resources and supportive services
- Document patient interactions and care activities accurately in the medical record system
- Promote compassionate communication and advocacy for patient and family needs throughout the care journey
